Role Description
This is a part-time, on-site/remote role located in the Fort Worth, TX area. This requires 10-14 hours per week.
The Registered Dietitian will be responsible for:
· Providing nutritional counseling and medical nutrition therapy to clients.
· Develop, implement, and monitor individualized care plans for residents of long-term care facilities using medical nutrition therapy.
· Collaborate and assist with culinary team to ensure protocols in place, and adherence to federal and state guidelines are being followed.
· Preforms menu review and approval, sanitation audits and assistance with regulatory surveys.
· Attend interdisciplinary meetings and rounds with clinical staff, as appropriate.
· Conducts admission, annual, and quarterly nutritional assessments to identify nutrition concerns.
· Serve as the lead dietitian for assigned facilities.
· Working in collaboration with the healthcare team to provide appropriate care and follow-up for clients.
Qualifications
- Dietetics and Nutrition skills
- Experience with nutritional counseling and medical nutrition therapy
- Weight Management skills
- Excellent communication and interpersonal skills
- Experience working with clients with chronic conditions, such as diabetes, heart disease, and renal disease
- Must hold RD credential through CDR and be licensed in the State of Texas.
- Ability to work collaboratively as a member of a healthcare team
